Dessa Simpson dbe7e4c52d Restructure DB in preparation for follower/subscriber implementation 
- Remove user.is{follower,subscriber}, replacing with tables follows and 
subscriptions
- Update vote_score_vw to reflect new structure
- Add db upgrade script
